A Study on the Main Issues and Improvements of Long-term Care Insurance in Korea - Focusing on the analytical frame of Gilbert and Terrell -

본 논문의 목적은 시행 4년이 지난 한국 노인장기요양보험이 당면한 주요한 문제점을 점검하고 그 개선방안을 모색하는 데 있다. 이를 위해서 길버트와 테렐이 제시한 할당, 재정, 급여, 전달체계의 네 측면을 중심으로 노인장기요양보험의 주요 이슈를 분석했다. 본 연구는 문헌고찰과 국민건강보험공단의 직원, 서비스 제공기관의 시설장, 보호자 등과의 심층인터뷰를 통해서 이뤄졌다. 연구결과에 따르면, 노인장기요양보험은 여러 문제에 직면해 있는 것으로 나타냈다. 할당측면에서는 낮은 보장성과 등급사정 기준의 비공개적인 변경 등의 문제가 있고, 재정측면에서는 중장기 재정의 급증 우려가 있는 것으로 분석됐다. 또, 급여의 측면에서는 요양보호사 인력의 대거 부족, 제공기관들의 폐업 등의 문제가 제기됐고, 전달체계에서는 사정에서부터 서비스에 접근해서 이용하는 등의 과정에서 각종 문제가 발생했다. 이를 해결하기 위해서 정부는 시장에 이들 문제를 맡기기보다 공공성에 기반한 적극적인 역할 수행이 필요한 것으로 분석됐다.
The aims of the research are to examine the issues of the Korean long-term care insurance and to find suggestions to improve the insurance since its approximately four years of implementation. For this, Gilbert and Terrell's analytical framework for social policy is used in terms of allocation, finance, benefits, and service delivery system. In terms of the methodology of the research, literature review and in-depth interviews with staff in the National Health Insurance Corporation, service providers, and carers were conducted. The findings of the research show that there have been a number of issues regarding the long-term care insurance. In terms of allocation of the insurance, there have been the issues of low coverage, closed revision of assessing criteria, and so on. In terms of finance, it has been predicted that there will be huge budget deficit of the insurance in the medium and long term. In terms of benefit, the issues of the shortage of care workers and the closure of service providers have emerged. In terms of service delivery system, there have been a number of issues in the processes of assessment and accessing and using services. Therefore, the Korean government should play an active role in tackling the problems rather than be left them to the market.
