초록
본 연구에서는 치매노인 가족부양자를 위한 집단프로그램이 가족들의 부양부담, 대처기술, 우울에 미치는 효과를 살펴보았다. 프로그램 내용은 치매노인 주부양자를 대상으로 교육 및 심리적 프로그램을 실시하여 치매, 치매노인, 부양부담에 따른 스트레스 등을 이해시키고, 대처기술을 강화시키는 데 초점을 두었다. 프로그램에 참여한 가족부양자는 실험집단 18명과 통제집단 21명이었다. 프로그램의 효과는 비동일 통제집단 디자인을 이용하여 프로그램에 참여한 실험집단과 참여하지 않은 통제집단의 사전 사후 부양부담, 대처기술, 우울을 측정하여 검증하였다. 그 결과 가족부양자 대상 집단교육프로그램은 참여자들의 부양부담 완화와 대처기술 향상에 부분적으로 긍정적인 영향을 미친 것으로 나타났다. 또한 실험집단은 사례관리 적용여부에 따라 하위집단으로 구분하여 프로그램의 효과를 검증하였는데, 사례관리서비스를 적용한 집단에서는 부양부담과 대처기술이 긍정적으로 유의미하게 나타나서 프로그램의 효과가 검증되었다. 특히 본 연구에서는 치매노인 가족부양자교육에 다학제적인 전문가가 참여하여, 교육메뉴얼을 통해 세 지역에서 동일한 교육을 진행함으로써 가족교육이 사회교육으로 발전할 수 있는 가능성을 제시하였다. 또한 집단프로그램에 사례관리서비스가 추가되면 프로그램의 효과를 더 높여줌을 확인하였다.
The purpose of this study was to examine the effect of the group education program on the caregiving burden, the coping skill, and the depression among the family caregivers caring for old people with dementia. A seven-week long education program was developed following the four specific objectives suggested by the MFW program incorporating multi-disciplinary areas. The eighteen caregivers participated in the group education program. The experimental group was divided into two groups and one of them were also provided with case management. A twenty-one caregivers in the control group were not provided with any service during the study observation program. A nonequivalent control group design was used to test the effectiveness of the intervention. ANCOVA analysis was employed to compare the pre and post values of the caregiving burden, the coping skills, and the depression between the participants and the non-participants in the intervention.
The results of this study found that the education program was partially associated with the reduction in the level of the caregiving burden and the improved level of the coping skills. The participants who were provided with the group education coupled with the case management showed the most positive outcomes in the caregiving burden and the coping skills.
This study suggested that the development of the group education program with multi-disciplinary areas be critical to the positive outcomes among the family caregivers. Also, a better outcome maintain with the group education program coupled with case management.
