저소득층의 의료 이용와 이용포기에 영향을 미치는 요인 - 경기지역 자활사업참여자를 중심으로 -

본 연구는 경기지역 자활사업참여자들을 분석대상으로 하여 저소득층의 건강과 의료이용실태, 그리고 의료이용 포기에 영향을 미치는 요인을 분석하였다. 본 연구의 분석 결과 저소득층의 전반적인 건강상태는 좋지 않았는데, 만성질환자의 비율이 약 40%로 높았고 주관적으로 인식한 건강상태도 좋지 않았다. 그럼에도 불구하고 저소득층의 의료이용 접근성에는 심각한 문제가 있음을 확인하였다. 만성질환자 중 27.2%가 현재 치료를 중단하거나 아예 받고 있지 않았으며, 응답자의 58.1%가 경제적인 이유로 의료기관이용을 포기한 경험이 있었다. 로지스틱 회귀분석 결과 저소득층의 의료서비스 이용포기현상은 주로 현재의 건강상태와 경제적 상황에 의해 설명되었다. 특히 경제적인 부담요인 때문에 저소득층이 의료이용을 포기하고 있다는 것은 정책적으로 중요한데, 이는 현재의 의료보장체계가 이러한 현상을 효과적으로 통제하지 못함을 의미하기 때문이다.
1989년 전국민 의료보장체계의 완성과 김대중 행정부 이후 진행된 보장성의 강화에도 불구하고 저소득층에게 의료에 대한 접근은 여전히 부담스러운 문제인 것으로 보인다. 따라서 본 연구의 분석결과는 현행 의료급여제도의 보장성이 강화되어야 함을 제시하고 있다. 이러한 맥락에서 지난 2월에 공포된 의료급여법 시행령 개정안은 우려요인을 갖는 것으로 평가될 수 있다. 의료남용 방지를 위해 소액의 본인부담금을 부과하는 것을 골자로 하는 개정안은 의료이용 포기현상을 심각하게 확대시킬 위험이 있기 때문이다. 이는 효율성의 제고라는 수단적 가치를 위해 저소득층의 건강권 보장이라는 본질적 목적을 훼손시키는 우를 범하게 될지도 모른다.
I analyzed, in this paper, the medical care utilization of the poor who particiapted in self-supporting program in Kyunggi-Province. 40% of the poor had chronic diseases, and self-evaluated health condition was not good. But, there were serious problems in access of the poor to medical care services. 27.2% of chronic patients did not use medical care sevices, and 58.1% of respondents had a experience that they gave up using the medical care because of economic burden. Logistic regression analysis demonstrated that the poor's abandonment of using medical care sevices were explained by health and economic burden. The findings suggest that current health security systems did not provide the poor with sufficient accessibility to medical care services.
Although health security system had covered all the people since 1989, access to medical care services seemed to be severe burdon to the poor. Consquently, it is necessary to raise up the benefit levels of current health security system, but government did not so. The government's actions in Feburary can make the poor's under-utilization of meical care be expanded, because consumer's burdens will increase. The implementation of this policy must be critically reconsidered.
