본 연구에서는 치매노인을 중심으로, 우리나라에서 실시되고 있는 중요한 재가복지사업의 하나인 노인 주간보호서비스의 비용효과 분석을 수행한 다음, 그 결과를 바탕으로 주요한 함의를 논의하였다. 본 조사는 유사실험설계 방식을 사용하여 치매노인을 위한 주간보호서비스를 이용하고 있는 주부양자 집단(처치조건)과 이용하고 있지 않은 주부양자 집단(통제조건)에 대하여 설문조사를 하였다. 4개월 간격으로 두 번의 면접조사를 실시하였다. 첫 번째 조사에서 처치조건에 속하는 171명의 주부양자와 통제조건에 속하는 117명의 주부양자에 대한 조사 자료를 수집하였고, 두 번째 조사에서는 처치조건 69명, 통제조건 65명의 주부양자에 대한 조사 자료를 수집하였다. 본 연구의 목적이 주간보호 프로그램의 비용효과성을 파악하는 것이기 때문에 두개의 패널이 만들어졌다. 이렇게 하여 처치조건의 65명과 통제조건의 61명에 대한 자료가 본 연구를 위하여 사용되었다. 연구결과와 함의는 다음과 같다. 첫째, 치매노인을 위한 주간보호서비스 이용은 치매와 관련된 증상들에 대하여 효과가 있었다. 둘째, 치매노인 주간서비스가 부양자의 스트레스요인과 심리적인 안녕의 특수한 차원을 경감시키는데 비용효과적이었다. 셋째, 사회적 비용측면에서 치매노인 주간보호서비스의 이용이 가정에서 거의 전적으로 치매노인을 보호하는 것보다 비용효과적이라는 것이 확인되었다. 넷째, 치매노인을 위한 주간보호서비스는 시설(병원이나 요양원)입소에 대한 징검다리로서 이러한 프로그램들을 이용하는 부양자에게 실용적인 선택방안이 될 수 있다.
The purpose of this study is to analyze the cost-effectiveness of day care programs for the elderly with dementia in Korea, and derive their implications using the results of analyses. A quasi-experimental design was used to compare two groups of primary caregivers of the demented elderly. Both groups were administered interviews at two intervals: first survey(baseline), 4 month later. In the first survey, data of 171 primary caregivers in the treatment condition and data of 117 primary caregivers in the control condition were collected. Four Months later, data of 69 caregivers in the treatment condition and data of 65 caregivers in the control condition were collected in second survey. Since the main objective of this study was to determine the cost-effectiveness of day care programs for the elderly with dementia, two panels were created. Therefore, data for 65 primary caregivers in the treatment condition and 61 primary caregivers in the control condition were used for this study. Findings and implications of this study are summarized as follows. First, the day care services for the elderly are cost-effective in relieving the symptoms associated with dementia or related disorders. Second, the day care services are cost-effective in relieving specific dimensions of caregiver stress and psychological well-being. Third, from social cost perspective, to utilize the day care services for the elderly with dementia are more cost-effective than to chose not to use them and rely on family care mostly. Fourth, the utilization of the day care services for the elderly with dementia may be practical option as a bridge to the institutionalizing an elderly relative(e.g., the placement of hospital)
