Factors Affecting the Choice of Medical Care Use by the Elderly Person

본 연구는 2005년 국민건강영양조사 제 3기 자료를 이용하여 노인들의 의료이용에 대해 분석하였으며, 그 초점은 의료이용의 미충족 현상과 의료보장제도의 영향에 맞추어졌다. 분석 결과 전반적으로 노인들의 건강상태가 좋지 않은 반면 경제력은 부족하여 의료수요는 높으나 이를 충족시킬 자원이 부족한 상황임을 알 수 있었다. 그럼에도 불구하고 실제 빈곤노인들의 의료기관이용은 일반노인보다 높게 나타나고 있다. 특히 의료급여대상자 1종의 경우 두드러지며 로지스틱 분석결과 의료급여 1종이 의료기관이용에 효과적으로 영향을 미치는 것으로 나타난다. 따라서 의료보장제도는 노인들, 특히 빈곤노인들의 자원부족을 효과적으로 보완하여 의료이용을 높이는데 긍정적인 기여를 한 것으로 평가할 수 있다. 그럼에도 불구하고 이러한 의료급여제도는 빈곤노인들의 의료수요를 충분히 충족시켜주는 데는 여전히 부족한 것으로 나타나고 있다. 노인들의 22.3%가 의료수요가 있음에도 불구하고 이용을 포기한 경험이 있으며, 그 이유 중 가장 큰 것은 경제적인 이유였다. 특히 빈곤층의 경우 경제적인 이유로 의료이용을 포기한 경험이 있는 노인들이 일반노인들에 비해 압도적으로 많았다. 이에 대한 계량분석결과 의료급여 1종과 2종, 차상위계층 등 모든 빈곤계층은 의료이용을 포기할 확률이 높은 것으로 제시되고 있다. 따라서 비급여항목의 축소, 본인부담의 완화 등과 같은 의료보장제도의 보장성 강화가 필요하다고 하겠다.
This study examines the use of medical care by the elderly person through analysing KNHANES Ⅲ databases, and the focus of the study is on under -satisfaction of medical needs and the impact of health care policy. The results of analysis are summerized as follows; the elderly person had generally suffered from poor health condition, and did not have economic resoure to satisfy the medical needs. But, poor elderly person, especially beneficiaries of medicare Ⅰ, have used much more medical care than non-poor. The result of logistic regression suggest that Medicare Ⅰ affected significantly on increase of uses. Consquently, the medicare system effectively made up the lack of economic resoure of poor elderly person.
However, Medicare did not sufficient to satisfy all the medical needs of the poor elderly person. 22.3% of the elderly person had experinced the abandonment of meical care uses, "the lack of econmic resource" was most important reason. The result of logistic regression suggest that all the poor elderly person such as Medicare Ⅰ and Medicare Ⅱ beneficiaries, and near-poor class had much more probabilities of giving up the use of medical care than non-poor. It is necessary to raise up the benefit level of current health care system such as the reduction of non-secured medical cost, the alleviation of user's burden etc.
