정신장애인을 위한 지역사회보호전략의 핵심 목표 중 하나는 지역사회통합이다. 이 연구는 지역사회에 거주하는 정신장애인의 지역사회통합에 영향을 미치는 요인들인 지지적 관계, 기관환경인식, 낙인지각, 그리고 자아존중감 사이의 경로를 살펴보았다. 특히, 자아존중감의 양면성이 지역사회통합에 어떠한 작용을 하는지 분석하였다. 이를 위하여 지역사회의 정신보건서비스 기관을 이용하는 총 432명의 정신장애인을 대상으로 설문조사를 실시하였으며, 구조방정식 모형을 이용하여 분석하였다.
주요 연구결과는 지역사회통합에 미치는 지지적 관계의 총효과가 가장 높았다. 자아존중감의 개념은 2가지 차원으로 구분될 수 있으며, 자아존중감의 긍정적 측면인 자존심을 통하여 가족 및 타인과의 지지적 관계가 지역사회통합에 간접적 영향을 미쳤다. 가족, 타인, 그리고 치료진과의 지지적 관계는 정신장애인의 낙인지각을 거쳐서 자아존중감의 부정적 측면인 자기비하감에 간접적 영향을 미쳤다. 연구결과를 토대로 정신장애인의 지역사회통합을 위한 실천적 함의를 제시하였다.
One of the critical goals of the community care system is to integrate the persons with psychiatric disabilities into the community. This study examines the process of associations among social support, the perception to the agency environment, perceived stigma, self-esteem, and community integration. A total of 432 clients who are utilizing the community-based mental health service agencies were analyzed, employing the Structural Equation Model.
Findings indicate that social support have the strongest effect on community integration. Two aspects of self-esteem are revealed. Self-worth mediated the relationship of social support from family and others to community integration. Perceived stigma mediated the relationship between social support from mental health professionals and self-deprecation. Social work implications of the findings were addressed.
