Cost-Effectiveness Analysis of Long Term Care Services: Care in Institutions vs In-home Care

본 연구는 우리나라의 가장 핵심적인 장기요양보호서비스인 방문요양서비스와 요양시설보호서비스에 대하여 비용？효과 분석을 수행하고 그 결과를 바탕으로 주요한 정책적 함의를 논의하고자 한다. 연구의 분석 표본은 방문요양서비스 이용자 38부, 노인요양시설 서비스 이용자 37부가 최종 선택되었다. 분석결과는 다음과 같았다. 첫째, 보호욕구가 강하여 요양등급이 높은 노인을 부양하는 부양자에게는 재가 서비스인 방문요양보다 시설보호인 노인요양시설이 스트레스 측면에서 더 효과적이다. 둘째, 스트레스 요인간 비용효과가 다르다. 1차적 스트레스 요인에서는 역할구속의 단위당 개선비용이 가장 낮았고 친교 상실이 단위당 개선비용이 가장 높았다. 2차 스트레스 요인 중에서는 근로갈등이 단위당 개선비용이 가장 낮았고 가족갈등이 단위당 개선비용이 가장 높았다. 셋째, 성과지표 전체에서 우울이 가장 비용？효과적이다. 종합적으로 검토하면 요양등급이 높은 노인을 부양하고 있는 가족은 노인요양시설서비스를 이용하는 것이 방문요양서비스 이용에 비하여 가족, 공적, 그리고 사회적 측면 모두에서 비용은 더 많이 발생시키겠지만 주부양자의 스트레스 요인들과 부정적 정신건강을 개선시키고 심리적 안녕을 증진하는데 효과적이다.
The purpose of this study is to analyze the cost-effectiveness of care service in institutions for elderly vs in-home care service represented long term care services in Korea, and derive their implications using the results of analyses.
For this research, primary caregivers that utilize in-home care service or care service in institutions for the elderly sanctioned as the elderly requiring institution care (care rank 1th or 2th) by National Health Insurance Corporation were drawn and surveyed. Among collected data, data for 38 primary caregivers utilizing in-home care service and 37 primary caregivers utilizing care service in institutions were used for this study. The results of this study can be summarized as follows.
First, for primary caregivers of the elderly with high dependence, care service in institutions for elderly is more effective than in-home care service.
Second, cost-effectiveness is different among stressors of primary caregiver. For costs spent to improve by 1 unit, there are role captivity the highest, next loss of intimate exchange, and role overload the lowest among primary stressors., and there were family conflict the highest, the next loss of self, and work conflict the lowest among secondary stressors.
Taken together, it cost higher from the family perspective, public perspective, and social perspective but is effective to improve stressors and negative mental health or psychology wellbeing that primary caregiver with the elderly sanctioned as the elderly requiring institution care (care rank 1th or 2th) by National Health Insurance Corporation utilizes care service in institutions rather than in-home care service
